Statement of Purpose
The aims and objective of the services provided by Forest Rose Care is to provide safe well led care & support to individuals with care and support needs to remain within their own home by working alongside outside services such as GP, District Nursing teams, Frailty and mobility services to promote person centred living for those who choose us to support them.
We aim to provide services such as personal care, promotion of independence, health and wellbeing support, medication management, companionship, outing and community support links such as providing support to enable engagement with local support or social groups to continue despite their need to be supported to promote and enhance mental wellbeing and encourage socialisation. We endeavour to listen & work with the individuals’ to meet and support their own preferences and choices of each client and provide bespoke care and support packages for their own personal needs & support clients to access services that will provide guidance and support with assessment , health reviews, and reviews of need so that they are able to have in place what they require to remain safe and be able to live well in their own home and be comfortable and safe within their own familiar environment. We plan to gain knowledge of each individual and work with them on personalised plans of care and support that are regularly monitored and reviewed for changes to promote ongoing wellbeing and to be able to gain or advise Clients to gain support of the relevant services when needed whilst ensuring the Clients safety so that Risk are limited , we do this by completing risk assessments and providing information support and access to outside services to gain knowledge information & equipment should it be needed to maintain safety or promote and individual to have all information they need on how they can limit their own risk to remain safe. We strive to provide care & support that is friendly, professional. compassionate & comforting to those in need.

Personal Care
We strive to promote a Person-Centred approach to the care we give. We Assist with Personal Hygiene, Activities & Medication, daily living engagement, support with rehabilitation after hospital stays, working with local district nursing teams to provide after care following their involvement
Companionship & Social Outings
Companionship care, Meaningful social interactions, emotionally & physically supporting with activities both inside the home and out in the community to promote mental wellbeing & physical health. Supporting with appointments & Meetings to work alongside relatives to create balanced support network for all
Household Assistance
Household assistance: Light housekeeping to include cleaning tasks such as vacuuming, laundry, Ironing & pet care. Meal preparation & supported food shopping. Safety checks to ensure we can create a safe environment which enhances independent living.
